I am on twice-daily mixtures of insulin and am flying to Los Angeles on holiday. How will the change in time affect my diabetes?
If your diabetes is not well controlled on your present insulin, this could be an opportunity to change to a more flexible insulin regime. However, if you are well controlled on premixed insulin, there are ways of adjusting your insulin to the conditions. It would be sensible to ask your GP for a supply of short-acting insulin (Humalog® or NovoRapid®) which you can take if your blood sugars start to rise. Mixed insulin, which is mainly long-acting insulin, is not effective at reducing the blood glucose rapidly. The advice about blood testing (see previous question) applies and if at any time your glucose rises above 12 mmol/litre you should take insulin every four hours, preferably before a meal. The dose will depend on how much insulin you normally take in a day. Once you have arrived, continue to take short-acting insulin every four to six hours until you reach a time when you would normally take your premixed insulin – usually breakfast and evening meal. You can then go back to your normal insulin doses, though you must continue to test frequently as holidays are notorious for upsetting control of blood glucose. For specific advice, you should speak to your GP or diabetes specialist nurse.
